- Type of service requested (domestic meter(s), fire service(s), irrigation meter(s), main line extension)
- Type of development (Residential Tract, Mixed-Use, Commercial, Industrial or Institutional)
- For residential tract, identify if development includes affordable low-income housing units, identify number of units and expected number of residents with affordable low-income housing units.
- For Commercial, Industrial or Institutional, identify CII classification. For more information on CII classification category please visit

Plans & Documents (Must Be to Scale)
- Architectural plans
- Plumbing plans
- Tract map, tentative tract map, or parcel map
- Storm drain plans
- Sanitary sewer plans
- Street improvement plans
- Grading plans
- Construction schedule
- Fire protection plan (if applicable)
- Landscape irrigation plans (if applicable)
Fire Protection Information (If Applicable)
- Scope of work (Public Fire Hydrant, Fire Line, Modify Existing Fire Service)
- Number of public and/or private fire hydrants (if applicable)
- Approved Fire Hydrant Location and Fire Flow Requirement Plan (if applicable)
- Identify if fire line is looped or pumped (if applicable)
- Total fire flow demand (in GPM @ 20 PSI and duration)
- Approved or preliminary fire department system plans and/or Fire Sprinkler plans. Approved plans must be submitted once available to complete the project
Landscape Irrigation Information (If Applicable)
- Total landscape area (sq. ft.)
- Maximum irrigation system demand (GPM)
- Estimated annual total water usage (ETWU)
- Irrigation plans
Notes:
- Recycled water is required for landscape irrigation if it is available within the vicinity of the project.
- For landscape irrigation systems using recycled water, plans and all required forms must be submitted to the Los Angeles County Department of Public Health (LACDPH) and the Los Angeles County Sanitation Districts (LACSD) for review and approval.
